Community Care Northumberland is set to significantly expand services for older adults following a major investment from the Ontario Trillium Foundation.
A representative from the office of Northumberland Peterborough South MPP David Piccini joined the organization Friday to recognize a $456,600 Grow grant that will support the expansion of its WrapAround for Older Adults program.
The three year funding will allow the program to transition from a volunteer led model to a staff led approach, improving consistency, capacity, and continuity of care for participants.
The WrapAround program provides person centred, team based planning to support older adults with complex needs. It connects individuals with services such as housing, transportation, healthcare access, and social supports.
Over the past four years, the program has supported 82 participants, with 34 successfully transitioning out after achieving positive outcomes. It is currently supporting 46 individuals.
With the new funding, the program is expected to support more than 100 older adults annually, reaching approximately 300 people over the three year period.
Community Care Northumberland says trained staff will each work with 30 to 35 participants, while volunteers will continue to contribute in mentorship roles.
Officials say the expansion will help address gaps in senior focused services in Northumberland County, ensuring more residents can access the care and support they need to live independently.
The Ontario Trillium Foundation says the funding is part of its broader investment in community programs that support health, well being, and connection across Ontario.
